UNDER 18’S TERMS AND CONDITIONS FOR YOUTH 2000 IRELAND RETREAT

PARENTAL / GUARDIAN CONSENT

It is important that you as parents/guardians understand what is involved with the retreat (see message to under 18s on reverse of this booking form). It is important that you read through this and talk to your son/daughter/child in your care about what’s involved. Parents/guardians must be available throughout the weekend to be contacted and possibly to collect your son/daughter should they want to go home or be sent home.

 

ALL terms and conditions must be agreed to when signing parent/legal guardian agreement with terms of retreat. 

 

  • I understand photographers and/video cameras will sometimes be present during the Youth 2000 retreat. I give permission for any resulting photographs or video, which may include my child, to be used by Youth 2000 for any promotional purposes on the website, television, or in newspapers, magazines or any other media deemed appropriate.

 

  • I/We, the undersigned parent(s)/legal guardian(s) of the identified minor, do hereby consent to his/her participation in the Youth 2000 Ireland Retreat and do forever release, acquit, discharge, and do hereby acknowledge that I/we do not have any cause of action against Youth 2000 Ireland in relation to all claims, demands, rights, and causes of action of whatever kind or nature, arising from and by reason of any and all known and unknown, foreseen and unforeseen bodily and personal injuries, damage to property and the consequences thereof, howsoever arising, and I/ we hereby indemnify Youth 2000 Ireland, its servants and/or its agents in this regard.

 

  • In the event that my child becomes ill or sustains injury while in the care of Youth 2000, I give permission to admit my child to any hospital for such treatment as deemed necessary.

 

  • I/We, the undersigned parent(s)/legal guardian(s) of the identified minor, do enclose a passport picture of the identified minor that will strictly be used by Youth 2000 Child Protection Officers only, so as to be able to identify the identified minor when required.

 

PLEASE NOTE:  As is specified in the terms above, a passport photo of the identified minor should accompany this form and will be strictly used by the Youth 2000 Child Protection Officers only, so as to be able to identify the minor among the crowds if necessary.

 

PLEASE NOTE:  If a child is in care/foster care, then TUSLA are the legal guardians and written consent from TUSLA is required.  Consent of foster carer is not valid.

 

PLEASE NOTE: If there is an organised group of 6 or more under 18’s, a responsible adult group leader must also be in attendance at the retreat.  If this is not possible contact Youth 2000 before the retreat.

 

 

MEDICATION:  Should the identified minor require medical assistance from the professional medical personnel present at the retreat, the undersigned parent/legal guardian will be contacted before any medication is administered.  However, should there be no answer or response by the undersigned parent(s)/legal guardian to Youth 2000’s efforts to contact on the contact information provided below, I/We, the undersigned parent(s)/legal guardian of the indentified minor, do hereby consent to the professional medical personnel administering of the appropriate and required amount of medication that the condition may require.  This may include:

Panadol, Nurofen, Nurofen-plus, Entonox gas for pain in injuries, Inhalers e.g, Salbutamol, (nebuliser) 0xygen,  Glucose gel, and Glucosamine for diabetics if needed.  Epin or ject adrenaline for allergies if needed.  Aspirin, Motilium, Motilium fast melt for gastric pain, Arret, Naloxone for diarrohea.

 

MESSAGE TO UNDER 18s:

Youth 2000 welcomes you to the retreat and we know you will fully partake in the entire weekend and enjoy yourself! The retreat will provide you with an opportunity meet new people, make friends and have the space to think about God.

 

It is important that you should have a clear idea of what goes on during the weekend and what your part is in the weekend. This is to be fair to you, to your parents/guardians and those who organise and run the retreat. Neither you, nor your parents/guardians, nor the volunteers involved want a situation where you come to the retreat with the wrong idea of what the weekend is about and the rules that need to be followed. So we’re going to go through what happens at the weekend and what the rules are.

 

WHAT’S GOING TO HAPPEN AT THE RETREAT?

The weekend’s main events will be daily Mass, talks, fun activities, music and singing etc. There will be time to socialise and hang out in the cafeteria and play games. There is also a reconciliation and healing service where you get to listen to encouraging reflections about the compassion and understanding of a loving God. You also get a chance to share your own views or thoughts in small groups if you want to share with others. But if you just want to listen that’s cool too.

 

SO WHY ALL THE RULES?

Because you are under 18, the people of Youth 2000 want to make sure you have an enjoyable and safe weekend. We also have a legal responsibility to look after your safety during your stay. This means that we have a Child Protection Policy which has been developed from “Children First” (Department of Health and Children, 1999).  We have to follow these rules in the interest of everyone involved – you, your parents/guardians, and the organisation Youth 2000. You will be allocated a guardian, someone over 18 of the same sex who has been checked by Youth 2000 that they are of good character and easy to work with. They are there to help you get the most out of the weekend and will help if you need anything.

 

THESE ARE THE RULES FOR ATTENDING THE RETREAT:

  • You must provide a valid form of ID and a passport photo for the Youth 2000 Child Protection Officers.. If you do not have a valid form of ID you will not be permitted entrance to the retreat.
  • No drugs or alcohol allowed. If you are suspected to be under the influence of any substance on arrival or over the weekend your parent/guardian will be called immediately to bring you home.
  • Boys must stay in boys allocated sleeping area and girls in the girls area. You must remain in this area after lights out each night until the next morning.
  • You are not allowed to leave the retreat area/premises or enter restricted areas. If leaving the retreat area you must be accompanied by your parent or someone your parent has permitted you to leave with. Your parent or legal guardian must provide written permission to clarify this.
  • Under 18s must meet up with their guardians at all ‘check-in’ times stated on the programme or at a specific time arranged with their guardian. In the case where you cannot make these meetings you must contact the CPO (Child Protection Officer) as soon as possible, failure to make ‘check-in’ times will result in the u18 being reported to the CPO as missing and necessary action will be taken. The last check-in for under18 is at 1pm on Sunday.  Parents/Guardians must arrange collection after this time, and this also includes if under 18 is travelling on a bus.
  • You must be contactable throughout the weekend by having your phone on and charged (just in case of an emergency)
  • You will be expected to attend all of the events over the weekend and to treat those around you with respect

 

Failure to follow these rules will result in you being given a warning and/or your parents/guardians being contacted to come and collect you.
